A. Please submit the following documentation with your application:
| i. | Two current testimonials from local church leaders or other ministering brethren who can attest to your character (NCIMF may send them a Confidential Questionnaire). |
| ii. | Two current testimonials from business persons outside of your local church who can attest to your standing in the community. |
| iii. | Submit either a current credit report or a testimonial verifying your financial integrity (NCIMF reserves the right to request an official Credit Report if needed.) |
| iv. | Two current passport size photos of yourself. |
| v. | One current photo showing husband/ wife/ family. |
B. Credentials/Ordination: if you have been previously credential through another organization please submit a photocopy of the certificate and explain your current status.
C. Have you read and can you agree with the Doctrinal Statement and Mission Statement of NCIMF with a clear conscience? zYes zNo (Comments are requested if needed.)
