N.C.I.U. AFFILIATE MEMBER AGREEMENT TO ACT AS TRAINING PROVIDER
New Covenant International University, a division of New Covenant International
Inc., an eleemosynary non-profit corporation organized and existing by virtue of the laws
of the State of Florida and of the United States of America, whose address is 7255 South
Military Trail, in the City of Lake Worth, Florida, hereinafter known as N.C.I.U.,
and __________________________________________________whose address is:

hereinafter known as "AFFILIATE,"
WITNESSETH:
Whereas, N.C.I.U. is organized for the purpose of training and educating Christian
leaders all over the world who are interested in spreading and preaching the Gospel of the
Kingdom to all nations and all peoples, in saving souls on a global basis and in
discipling men and women so that they may all come into the perfection of Christ; and
Whereas, N.C.I.U. is established as a Christian University under Florida Statute,
with affiliated schools, colleges, and theological seminaries all over the world; and
Whereas, N.C.I.U. is interested in both establishing new Bible Schools, Colleges
and Theological Seminaries, and in assisting already-existing Bible Schools, Colleges, and
Theological Seminaries who desire to become and affiliate of N.C.I. U.; and
Whereas, N.C.I.U. is willing to accept AFFILIATE as an Affiliated Training
Provider of N.C.I.U.; and
Whereas, AFFILIATE desires to be an affiliate member of N.C.I.U.
Now therefore, N.C.I.U. agrees to accept AFFILIATE, and AFFILIATE
agrees to become an Affiliated Training Provider in accordance with "Schedule A"
attached hereto and made a part thereof.

"SCHEDULE A"
1. N.C.I.U. will require that AFFILIATE make application for
membership with New Covenant International Ministries Fellowship as an Affiliate Training
Provider, and that the following documentary evidence be submitted with this application
for affiliation:

A certified copy of the Constitution, By-laws, and/or
Articles of Association |
|
[b] |
A certified copy of the determination of Incorporation and
Exemption Status (if applicable) of the organization |
|
[c] |
A statement from the president/director setting out the
history of the organization, including the current vision statement |
|
[d] |
A profile of the organization showing the governmental
structure and management policies |
|
[e] |
A copy of the corporate resolution agreeing to affiliate with
N.C.I.U. as an academic institution, and NCIMF as a Ministries Fellowship |
|
[f] |
A letter from a Certified Accountant, or evidence by way of
audited accounts, stating that the organization is not in fiscal delinquency, and that
standard accounting practices are upheld and practiced |
|
[g] |
A sample of the current brochures utilized by the
organization in advertising their institution |
|
[h] |
Details of any other affiliations held by the applicant
organization |
2. N.C.I.U. requires that the AFFILIATE supply the following
information to N.C.I.U. for evaluation of the institution, curriculum and faculty of
Affiliate's organization as an Affiliated Training Provider:

[a] |
Full details of all courses and programs provided by
Affiliate's institution. This information is intended to give an indication of the
organization's overall involvement in providing systematic education and training. |

[b] |
Staff employment policies and practice: The following is
required -- A statement outlining the policy and procedure of staff recruitment and
employment. -- Staff records that include appropriate curricula vitae.- A statement on
Staff development and updating of skills/qualifications. |
|
[c] |
A statement of the educational philosophy and/or mission
regarding what the institution intends to achieve educationally, and any relevant goals
that have been established. |
|
[d] |
Current accreditation/recognition status: If the programs,
courses and institute have been approved by any acccrediting body, educational
institution, professional registration body, or other organization, then this information
should be made known to N.C.I.U., including current status or reasons for discontinuing. |
|
[e] |
Assessment and recognition of prior learning: a policy
statement is required from the affiliate's organization regarding any policy held or
implemented regarding prior learning assessment and recognition. |
|
[f] |
Student Record Keeping: A statement is required detailing the
current system of student record keeping employed by the organization. A sample of a
student's record is required for examination. The minimum standard of acceptable record
keeping is a primary record that identifies the course or program, the provider, the
student/trainee, the basis for admission to a course or program, and a description of the
education or training undertaken (i.e., the competencies and results achieved). |

Facilities, accommodation, student services and amenities
available: the following information is required where applicable -- Indicate the relevant
major teaching resources and facilities available at the institution, including major
items of equipment and resources.
 | An appropriate list of library holdings should be available if required, or a statement
concerning the plans for establishing a library given. |
 | Accommodation, such as lecture rooms, workshops, administration rooms etc., should be
detailed with a plan of the facility where applicable. |
 | Details if applicable of student accommodation, or accommodation policy if off-campus. |

Course/Programs, Curriculum Development and Advisory
Consultant Policies: A statement is required detailing the methods used in establishing
the course/programs adopted by the institution, and explaining how the current curriculum
is determined, with details of any Advisory/Consultancy body established for overseeing
future development. |
3. N.C.I.U. further requires that during the course of this agreement,
that AFFILIATE and N.C.I.U. work at maintaining a relationship in accord
with biblical principles of stewardship and harmony that gives credence to the title of
"Christian organization," and that AFFILIATE purposes to:

[a] |
Hold and maintain all records and files on behalf of N.C.I.U.
in a prescribed manner, and that all such records pertaining to students admitted to the
N.C.I.U. programs are the property of N.C.I.U., and to surrender them to N.C.I.U. with
full details of course/grade results when required for graduating the student, or for
transcription and record purposes, or at the termination of this agreement. This includes
all thesis/dissertation and project works undertaken. |

[b] |
Receive and handle all monies collected on behalf of N.C.I.U.
as "trust monies", and deposit them in the nominated accounts as directed,
giving full account of monies by way of prescribed statements and reconciliation. |
|
[c] |
Keeping as confidential all fiscal policies in respect of
agents' commissions, student financial sponsorships, and any other such information given
to affiliate as a member. |
|
[d] |
Adhere to the correct legal position in all advertising
concerning the source of Certificates, Diplomas and Degrees being issued, by acknowledging
the origin and statute under which N.C.I.U. issues these awards, namely that, "Member
... is affiliated with N.C.I.U. who is authorized to issue these awards by the State Board
of Independent Colleges and Universities, under Florida Statute 246.083." |

[e] |
Ensure that as an affiliated organization they do not make
commitments or give special conditions by way of alteration or variation to prescribed
academic pathways, courses or programs for students unless with express written consent
and permission from authorized N.C.I.U. officers, thus preserving the integrity of the
programs. |
|
[f] |
Ensure that they do not give any misleading advertising
whether written or implied, that would lead the student to conclude they have an automatic
right of entrance into any other institution of higher learning having once earned an
academic award through N.C.I.U., but that they would acknowledge that such prerogative is
held by the receiving institution alone. |
|
[g] |
Act and conduct their affairs as an organization, in such a
manner that will not bring reproach either on the name of Christ as Lord of the Church, or
N.C.I.U. as an institution. |
|
[h] |
Allowing N.C.I.U. to send a faculty member, Board member, or
other suitable representative to visit Affiliate's institution and facilities as required,
and to give reasonable access for the inspection of records and files pertaining to the
affairs of N.C.I.U. as conducted by Affiliate, at N.C.I.U.'s expense. |

[i] |
Making provision for the voluntary interchange of faculty,
tutors, students and other members as required with other global affiliate members for the
extension and enrichment of the global network of schools, colleges and theological
seminaries. |
4. N.C.I.U. therefore commits itself to working with AFFILIATE for the
aims and objectives stated in the Vision and Missions Statement by:

[a] |
Providing an approved academic system and programs that are
culturally and spiritually relevant for the student body enabling them to receive
appropriate awards and qualifications to fulfill the Great Commission and the Vision and
Mission Statements of N.C.I.U. as given in the current catalogue and brochures. |
|
[b] |
Continuing to maintain academic consultation with both
suitable academic personnel, end-user organizations and students, that will result in a
contemporary curriculum being taught and mentored. |
|
[c] |
Submitting to and maintaining the standards as set by the
Florida State Board of Independent Colleges and Universities, and any other recognized
issuing or accreditation bodies with whom N.C.I.U. is associated or registered, that
result in the right to issue such academic awards including, but not restricted to
Certificates, Diplomas and Degrees and qualifications as approved and advertised by
N.C.I.U. in their current catalogue and brochures. |

[d] |
Maintaining an adequate and efficient system of record and
data storage information to enable the provision of transcripts and academic records for
students each year, as required. |
|
[e] |
Maintaining an adequate supply of courses, programs and texts
for the certificate, diploma and degree courses offered. |
|
[f] |
Providing each affiliated member with a Master copy of the
current catalogue and Handbook, including all sample forms and addenda. |
|
[g] |
Providing each affiliated member with an annual list of all
graduating students. |
|
[h] |
Assisting with the provision of such faculty members, tutors
and representatives as to enable member to hold special convocations or tutorial seminars
at member's expense. |
|
[i] |
Providing each member with an annual directory of all other
schools, colleges and theological seminaries that are members of the N.C.I.U. global
network. |
|
[j] |
Endeavoring to maintain the unity of the Spirit with all
members and to build up each institution by various means as led by the Holy Spirit,
recognizing that the God who calls us, equips and empowers each of us with different
gifting and abilities, but that it is the same God who works in us and through us all. |
